Please complete this questionnaire to the best of your ability if your fish are sick (copy and paste). The more details you can provide, the better we are able to diagnose and help you treat your sick fish. IF you have any questions about the information in this document please feel free to ask. If possible, try to provide us with pictures or video as well.
It may seem to some that all the question we ask are annoying or unnecessary. However there are so many factors that go into what may be affecting your fish. It may seem silly or stupid to ask all these questions about something that seems strait forward or asking to see a picture. However any number of things could be the reason; water temperature (too cold or hot), ammonia / nitrite / nitrate levels, over feeding, dirty water, water that's has too high or low ph levels, a parasite, bacterial infection or something else. Getting a visual with all the facts can help other people pin point the reason and get your guy feeling better faster or even be the difference between a healthy fish and one that doesn't make it. When it comes to a sick fish detective work is your best friend.
DISEASE QUESTIONNAIRE
Problem:
1. Please explain the problems with your fish. When did you notice the problems and did anything unusual happen that you think started them? How long have you had the current fish/betta?
2. Fish Symptoms; i.e. turning dark, colour change, excess slime, not eating, clamped fins, flashing (scratching on tank decore etc.), darting, clamped gills, white/yellow/green poop, hiding, headstanding or tailstanding, white on tips of fins, rotting or fungus, blisters/white zits on fish, bloated, cloudy eyes, wounds and any other symptom not listed here.
3. What medications/ treatments have you already tried and what were the results. Include dosage and duration of treatment.
4. Include any pictures or videos you have which shows the symptoms. If you can't add them to this post, please provide a link to them. It'll help others to more accurately understand what's going on and give advice.

5. Tank size and ages, numbers and sizes or types of fish? What type of filtration do you have?
6. Water Parameters and temperature; Do you have a test kit and if so what are your numbers for ammonia, nitrites, and nitrates and any other test you feel is relivent?
Note: Water Parameters are important in diagnosing problems within a tank. If you don't own test kits for the following information, you can purchase them or take your tank water to one of many fish stores for complimentary testing. Test your parameters and post this info as soon as possible because levels can affect your fishes health.
